Create Test/Survey Month values missing

| Operating system ATutor is installed on - linux ATutor version - 1.6.2 Patch #s applied - all to # 21 ATutor theme name - 1.5 PHP version - 5.2.9 MySQL version - 5.0.x community Webserver & version - Opera 9.64 Create Test/Survey - Start Date and End Date are entry select boxes separated into day, month and year. The month list has just square brackets [] as content for each of 12 entries in list. Should have the name of each month. Viewing page source shows there are just [] in each select option so problem is not browser related. Possibly a result of changes introduced in patch 21. I included all patches including #21. The first time I tried to create a course, I found that the month values were missing for the Release Date and End Date. I renamed /include/lib/output.inc.php to output.inc.php.0021.new then renamed /include/lib/output.inc.php.0021.old to output.inc.php When I tried to create a course, the month values were shown correctly.
